Survey on Long COVID Health Services for Adults in England

About the survey

Long COVID Support  a not-for-profit organisation, advocating for those impacted by Long Covid is conducting a survey on Long COVID Health services in England.

They are keen to hear experiences of Long Covid service(s) for adults (those aged 18 years +)  in England.

You can do the survey for yourself, or on behalf of a friend or relative who has Long Covid. If you are answering on behalf of someone else, answer the questions as though you were that person.

This survey has been designed and developed by people who have Long Covid.

The survey will take 15 - 30 minutes to complete depending on your answers. You may save the survey and finish it later as long as you use the same device to complete it.

The data gathered from this will be discussed with NHS England through the NHS England Long Covid Taskforce. Several Trustees and members of Long Covid Support are on this Taskforce.
